A case of renal stones with Gouty disorder

       A 23 years old male complained of sharp stitching and tearing pain in renal angle on both sides on and off since 15days. He has consulted an urologist in July 2010. Ultra sonograph examination revealed bilateral renal calculi (Refer the plate I) and blood uric acid level was 7.2 mg/dl. He was under allopathic management for 3 months without appreciable improvement. He has reported to our clinical unit with the same nature of suffering and intensity, and was treated on Homoeopathic principles and followed up for 4 months. He not only got total relief of renal colic along with associated problems symptomatically but also Ultra sonographic report revealed normal study (Refer the plate II) and blood uric acid level has decreased (4.2 md/dl).

Chief complaint
• Renal colic since 3½ months
• Urticarial rash on and off since 1 year
• Swelling and pain in the right ankle and big toe since 1 year
• Itching in genital region since 6 months

H/o Present complaint

   Patient complaining of stitching and tearing pain in renal angle on both sides on and off since 3½ months. Sometimes experiencing sharp pain. Urticarial rash on and off since 1 year < sea fish, brinjal and chicken curry. He was using ntihistamine drugs (allopathy) as and when required. Stitching and tearing pain in the right ankle and big toe with swelling
on and off since 1 year < exertion, night and fatty foods. He was detected to have higher blood uric acid level (7.2 mg/
dl). Complaining of Itching in genital region since 6 months < night, sweating.

Past History

          Had jaundice at the age of 12 years.

Family history

          Father is hypertensive; mother has gouty joint pains.

Physical generals

Complexion - White colour
Appetite - Normal
Thirst - Normal

Bowel movements - Regular
Urine - 8-10 / 2-3 D/N; frequent with sudden urge to pass urine on and off, some times complaining of sharp pain at tip of penis after urination.
Desire - warm drinks

Mental Generals

• Much worried of his medical problem.

General examination

• Weight : 62 kgs
• B.P 120/80 mm Hg,
• PR: 74/min,
• CVS: S1,S2 normal
• RS:NAD
• P/A : Slight dull note at renal angles; Slight tenderness in the lower abdomen

Investigation

The ultra sonogram of the abdomen – bilateral lower lobe tiny calculi (Refer Plate - I ), Blood uric acid level - 7.2mg/dl.

First Prescription                      16/10/10
Rx
      Urtica urens Q 20 drops tid for 1 month

Follow Up 1                               20/11/10
      Renal colic completely reduced; patient has experienced of passing stones during urination 10days back with haematuria and sharp pain at tip of penis; Urticarial rash, swelling and pain in the right ankle and big toe,- reduced to 25%; Itching in genital region – status quo. He was asked for the ultrasonograph of the abdomen on 6/11/10 – no renal stone seen in KUB region but right kidney showed grade –I hydronephrosis - Urtica urens 0/3 tid for 15 days; SL for 15
days

Follow Up 2                              15/12/10

      No renal colic and other associated symptoms reduced to 50% - Urtica urens 0/6 tid for 15 days; SL for 15 days.

Follow Up 3                              17/01/11

       No renal colic and other associated symptoms reduced to 75% - Urtica urens 0/10 tid for 15 days; SL for 15 days.

Follow Up 4                              10/02/11

      No renal colic and all other associated symptoms reduced totally - S.L-30 days.
Investigation

      The ultra sonogram of the abdomen – Normal study (Refer plate II)
      X-ray –KUB- Normal study Blood uric acid level – 4.2 mg/dl

Discussion and conclusion

     Urtica urens was selected on the basis of totality and given in mother tincture initially. This remedy is stand tested in mother tincture as ‘wash out’ remedy for renal stones with uric acid diathesis. This is verified in our study. The same medicine when used in 50 millisimal 0/3 - 0/10 in repeated doses has not only helped in relieving the renal colic completely but also reduced the other associated symptoms like Urticarial rash, gouty swelling and genital itching.
