     Dr. Anitha Aswini,Chennai presents a case and seeks advice and remedy

A 58 years old fair ,obese widow consulted me for pain in the left shoulder; Left arm, Left forearm, since one year. Aching type, Not radiating, >lying on right side, >sleep; Restriction of movement above 90°.Additionally she had pain and stiffness in the right middle finger. (Especially middle phalanx) > knuckles. She also suffered from sleeplessness in midnight especially after 11pm. disturbed from least sounds and sensitive to noise.

      She is a Diabetic and under Insulin twice daily. Post prandial sugar 219 mg/dl. She has insecure feeling and fears about the presenting complaints, and thinks that it will lead to paralysis. She likes to do physical work than the mental work, Hot patient, having profuse sweat on the head and other generals are normal.

     First I started with Sulph 200 and Passiflora Q was prescribed to encounter insomnia. . Patient feels somewhat better then after 15 days, I gave kali carb200, after that she had Dry cough at bedtime, Which also disturbs the sleep. Finally I gave Cal. carb200, now patient feels better. Not permanent cure. Please tell me if the Medicines given are correct or what potency.Should I follow, please guide me.

     Dr. S. Mohamed Aleem is a very leading practitioner practices at Chennai, he find remedy to the case

     This case exhibits only partial picture of the patient. The given symptoms per se, the partial totality, point mainly to Sulphur. In my experience, Sulphur is a good medicine for arthritis especially for osteoarthritis. Though there are some mental symptoms like insecurity and fear, for this case they are common symptoms only. In the absence of characteristic mental symptoms, it is better to prescribe Sulphur in low potency either in 6 or 30 and to be repeated daily twice or thrice till some change - either for worse or for better - take place.

     Prescribing on the basis of aetiology, miasms and pathology also may be needed if the case becomes refractive to the so-called indicated remedy - Sulphur, in this case.

     If Sulphur low potency does not give any improvement, retaking the case and understanding the patient's past history especially menstrual history, past life situation and present life situation and her temperament may give some ideas in relation to aetiology and miasm. The effect of bereavement (she being a widow) and the effect of dependency (if she is dependent on others) also have to be studied to get the clues for the right remedy.

     It is my suggestion to consider Natrum sulph. on the basis of sycotic nature, obesity and diabetes.
